I wouldn't discount the tried and true -YouTube. There are ways to tweak it and options in the settings that allow for some good flexibility. It fits almost all of your criterion and it's wicked easy to use and obviously has universal playback...
sure there are drawbacks but ...

I have uploaded 291 videos that have been viewed 874,000 times since I started back in 2006... so dag-gummit, I guess I know what I'm talking about :)
