David, CISCO Unified MeetingPlace does everything you are looking for below. I have never had any problems with it. That being said, I am not in the small business/self-employed area as I'm guessing most on this list are (I am with one of the large companies in Hartford)...I am saying this because I have no idea on how pricy that option is.
