J., There are several programs out there which can do that. While I haven't used it in production yet, Articulate's Quizmaker seems to be one of the nicer ones: http://www.articulate.com/products/quizmaker.php The obvious limitation is in how much you can skin the interface and if you need some kind of unusual question setup. If you want something super-slick, you'd probably want a custom job. It does have a decent clean look by default, but be sure to turn off or change those horrible sound effects it defaults to. You also need to keep in mind what you want to do as far as tracking the results. They have their own online service you can tie into or it'll export to SCORM for an LMS.
