Thanks to everyone who wrote. The answer is that the current Mac version of Adobe PDF Maker (and Acrobat) does not preserve internal links or have the "Adobe PDF" menu that appears in Word for Windows which allows you to adjust the PDF Conversion settings (including telling it to include links) http://www.adobe.com/support/techdocs/325287.html I'll just need to do it on a Windows machine once I've got all the graphics completed.
