What was that commercial on t.v. all the time a while ago?
Forget the name, but this was one of those portable scanners that indexed your docs for you.... but I believe there was a manual process to enter the information

I use Epson GT-S50 (a bit old now) for bulk scanning of sheet feed documents, the software with it does a good job of OCR, so the PDF produced have text and the files are named in sequential order with a fair bit of control of root filename.

if you feed each separately you will get one PDF per document from one front panel (of
scanner) button press

How you pull the name is a function of the format of the text, I just finished scanning and loading into a database 450 biographic sheets and suplimental documents with image
  (over 6 binders worth)

I used pdftotext  to create formatted text from the pdf and the perl script to populate the database fields.
pdftotext is open source with window, mac or linux versions)

You could use a similar scheme to rename the files per whatever you needed
