Hi Marcia,

I understand that PDFPen  has that ability. 
http://smilesoftware.com/PDFpen/index.html (if your student has a Mac or 
iPad)  I'll be interested to see what others have to say.

I would think that any forms building you do to meet his need would end 
up being unsatisfactory. How do you know where he wants to take notes?
