For most of *rural* America, dialup is pretty much the only option (and
for some of us satelite (or celluar) internet is not an option, usually
for topologial reasons (mountains and/or trees in inopertune places). 
The US is *16th* in Broadband Internet availablity (behind several
'third world' countries!).  Embarassing but true.
