HI Kimo, Yes, that's a great help. I also got some comments off the list. I think what I'm asking for is as you say "fairly straightforward forms work." I'm going to create the storyboard before I do anything more. It's not my intention to do any detailed sociological analysis, though I should capture the data in case there's something interesting. The questionnaire if for users, and is a service to them. It will also be a reason to come to the blog site.
