Hi Annamarie,

I think the scope of your project depends upon what you mean by  
"summarized". If you just need a multipage form that formats the  
answers and sends the data as text in an email, yes, that's fairly  
straightforward forms work.

If you need the data analyzed based on some kind of subjective  
criteria then that would be an entirely different kind of project.

In the case of Likert Scale questions, this might start with the  
dynamic creation of graphics illustrating the positioning of the  
subject's answers along a horizontal line -- from Strongly Agree to  
Strongly Disagree.

If you needed to then summarize and analyze groups of answers, that  
would combine programming with some kind of sociological knowledge of  
the subject matter, which I assume is your "thing".

Longstory short, you could look for people with PHP or ASP, and CSS  
skills to start and maybe a designer to lay it out so it looks nice.  
How far you take the data analysis and the presentation of it is up to  
you and your budget.
