I have been useing Futurequest (based in Florida) for several years now,
after having hosted with several local services. They are not the lowest
cost around ($9.95/mo for the cheapest package), but have an excellent
management system and support, along with an active users forum, and I
have found them to be extremely reliable.

All their packages seem to have everything you are asking for. You can
find out more at: http://www.futurequest.net/Services/Packages/
