What are the forms settings? Anything that requires persistence past the 
form submission may trigger it.

Settings that may require the Google account include Collecting email 
addresses and Limiting to 1 response. Other restrictions (i.e. to the 
trusted org) may also be a cause.

I also recall use of an email field had some gotchas essentially because 
Google doesn't trust form creators to not misuse emails.
