Hi Don,

I think Ben is right that the least technical way is to use google forms.
However PDF can do form submits to a webhook fairly simply Google Results

A webhook can be setup on a webservice, or you can use a tool like Zapier
to receive the data and tie into any other existing workflow that needs to
happen. It really depends on where your customer is at in terms of what is
built up around this form already.
