Hi, Marcia,

Instead of an online form, I email my questionnaire to prospects
directly--with different versions for different market niches (e.g., book
marketing). Here's the one for green marketing.

I'm sure the list processor will make a hash of it, so I'll also send you
the original Word doc to your personal email.
