[Hidden-tech] How? Who? What do I need?

Of course I know about Survey Monkey and Wufoo. Those are SURVEYS, the responses are aggregates. I need something more like a personality assessment. The individual gets only their own answers. Why? Because it helps them ID what they already know, but don't have in B&W. 

With that additional clarification - any one got an idea?
