While I have (and use) Spam Assassin on my server as a first line of defense, I also use Mailwasher on my desktop and highly recommend it. Mailwasher (http://www.mailwasher.net/) comes in a free version which has nearly all the capability of the paid version except that it only handles a single mail account.
