Those symptoms are similar to the old problem of the "desktop database" being corrupted. the simple and effective solution was to "rebuild the desktop" by restarting using a key combination thatr forced a rebuild of that database. I'm not sure if there is a counterpart to that problem in the current versions of the Mac O/S but it might be worth Googling "Mac desktop" and that sort of thing to see if it is still an issue.
