The information about this is laid out quite clearly on the State Dept web site - I was reading about it a few weeks ago. My recollection is that possession of a ticket for international travel in less than 2 weeks in hand for a visit to the Boston office is the way - no appt needed, from my recollection, but double-check on the state.gov site, which covered a number of scenarios in great detail and specificity.
