Will,

I'll assume for a moment that you are on Windows.  BCL's EasyPDF  
printer driver should work perfectly for you, and the price is right:

It can convert MS Word styles into PDF bookmarks.  I know that it  
also has hyperlink export support.  However, I'm on OS X at the  
moment, so I can't verify whether it will preserve those internal  
links (as opposed to only preserving "outgoing" web hyperlinks).
